Consumer Information & Records Requests

In accordance with Texas House Bill 4224 and Section 181.105 of the Texas Health and Safety Code, this practice, HealingChoice Family Therapy is providing the following information to our patients:

1. How to Request Your Health Care Records

To request a copy of your health care records from this facility, please follow these steps:

• Submit a Written Request: You may request your records by completing our "Release of Information" form, available from your Clinician.
• Delivery Method: You may choose to receive your records via Secure Patient Portal, Encrypted Email, or Printed Paper.

We will process your request within 15 business days as required by Texas law. If you have any questions please direct them to your Clinician.

If you have questions or concerns regarding the licensing or professional conduct of your practitioner, you may contact the Texas Behavioral Health Executive Council (BHEC):
